All partners will be required to:

• Collect demographic data on individuals reached and submit reports on a schedule specified by HHF.
• Dedicate key staff/volunteers who will provide administrative support to implement the project.
• Spend funds in accordance with the contract award.
• Successfully provide services according to criteria.
• Participation in conference calls, virtual meetings, in-person meetings, and walk-throughs for program evaluation.

Metrics to track:

• Expanded collaborations
• Responsiveness of population served
• Number of expanded collaborations
• Number of newly initiated collaborations
• Number of educational sessions hosted
• Number of outreach activities completed
• Number of unduplicated persons served

Responsiveness to the invitation (Pass/Fail)

• The proposal shall be responsive to all material requirements, enabling a proper and fair evaluation according to the Houston Health Foundation evaluation criteria.

Financial Stability of the Proposer (Pass/Fail)

• Proposer shall submit the most recent income statement and balance sheet.
• A full disclosure of any events, liabilities, or contingent liabilities that could affect the Proposer’s financial ability to perform this contract.

Background and Organization Capacity (10 points)

• Proposers shall briefly describe the organization’s mission and services, including how the proposed program fits into current operations.

Service Delivery Experience (10 points)

• Program data and examples that demonstrate community impact
• Undertaking an effort to change organizational culture
• Developing program implementation plans
• Working with other organizations.

Program Summary (10 points)

• Proposer shall provide a summary of how they will provide the services required

Program Design and Implementation (25 points)

• Proposer shall describe program design and implementation plan, including how the program plans to deliver work.

Program Management (10 points)

• Proposers shall describe how the organization will identify and recruit key staff and/or volunteers required to implement the program.

Program Tracking and Reporting (10 points)

• Proposers shall describe your process for ensuring consistency and accuracy of data collection

Implementation Timeline (15 points)

Proposer’s timeline shall include, at a minimum following:

• Startup/program milestone
• Activities
• Staff Responsible
• Projected start date of activities
• Projected date of milestone completion.

Budget (15)

• Propose the budget needed to perform RFA criteria successfully. Include a brief narrative for each line item. The foundation will only consider requests up to $50,000. This budget demonstrates financial accountability and should be itemized over a 10 - month cycle in proportion to the requested amount. Internal budget allocation should not exceed $5000 per month to perform services.
